Health Information Management (HIM), also known as Medical Records, is responsible for cataloging and maintaining the medical records of each and every patient treated at Twin Lakes Regional Medical Center. Our staff is committed to ensuring the medical records of all patients are complete and that the information contained within is kept private.
Medical Records from physician offices - Twin Lakes Regional Medical Center does not have access to a patient's medical records created and stored by their physician. Patients must contact their individual providers for records from doctor's offices.
Twin Lakes Patient Portal has changed to MyChartWe are no longer adding new accounts in the legacy patient portal. Current account holders will be able to log in and access their previous medical records until September 30, 2021, or call 270-259-9517 to request a copy.

ROI services provides health information to patients, physicians, other healthcare providers, attorneys, insurance companies and other parties with a need to obtain copies of patient records. With the introduction of the Health Insurance Portability and Accountability Act in 1996 (HIPAA) the rules and regulations regarding releasing Protected Health Information became more stringent and a great deal of expertise is required to comply with the laws. The Authorization to Release Protected Health Information should be filled out completely and mailed in or brought to the Health Information Management Department in person. Your photo identification is required to obtain copies of records. Please call the ROI desk with any questions.

The Director of HIM is certified in Healthcare Privacy for Security by the AHIMA and serves as the Privacy Officer for TLRMC, investigating and resolving any complaints by patients of a breech of privacy of their Protected Health Information. They also handle all requests for an amendment to medical records in the case that a patient does not agree with the physician documentation on their chart.
