SOCIAL WORKER/DISCHARGE PLANNER

Twin Lake Regional Medical Center’s Social Worker/Discharge Planner is trained to help patients and family members deal with financial, social and emotional problems that relate to illness or hospitalization.

The Social Worker is involved in discharge planning and works with patients and families to help deal with long-term illnesses and rehabilitation.
